I. Overview

The performance of every employee of the district shall be formally evaluated periodically as outlined in Administrative Policy #113.

II. Confidentiality

All employee evaluation records are confidential, and shall be treated as outlined in Administrative Policy #111, Employee Record Confidentiality.

II. Procedure

A. Supervisors will obtain the proper evaluation form for the position the employee holds in district service.

B. A supervisor shall check out a copy of the previous evaluation conducted for the employee to use as a point of reference, if the same supervisor wrote the previous evaluation.

1. The supervisor can obtain a copy of the previous evaluation from the Administrative Assistant, the Assistant Chief or the Fire Chief. The supervisor must sign for the copy.

2. The copy is to be returned to the Administrative Assistant, the Assistant Chief or the Fire Chief for destruction.

3. The contents of the personnel evaluation shall not be shared with anyone other than the employee involved or a chief officer.

4. The original evaluation shall remain in the individual's personnel folder (except for copying).

C. The supervisor will complete the evaluation form according to the directions contained therein as honestly and completely as possible. No other person can complete or in any way have input to or view the evaluation during the process except the involved employee, the supervisor writing the evaluation, or a chief officer of the district.

D. Prior to discussing the evaluation with the employee, the supervisor shall confer with the next level chief officer regarding the content of the evaluation. The chief officer has the authority to alter selected areas of the evaluation with justification.

E. The supervisor will then discuss the evaluation with the employee in private, and allow the employee to have input both verbally and in writing on the appropriate places on the form.

F. The supervisor and the employee shall both sign and date the form.

G. The supervisor shall then submit the evaluation to the next level chief officer for the chief officer's signature and placement in the employee's personnel file
